The real question: "My son twisted his ankle while playing, and he started crying. Should I go for an X-ray immediately?"
The answer: You don't always need an X-ray. Here are 3 signs to help you distinguish before reaching the hospital.
Ottawa Ankle Rules — Adapted for Dubai
If the patient can:
- Walk 4 steps (even with pain)
- No pain when pressing the two prominent bones (malleoli)
- No pain when pressing the base of the fifth metatarsal bone
→ Probability of fracture <1% — rest and cold compress are sufficient.
Important note: In Dubai's hot climate, swelling appears faster — its severity does not necessarily indicate a fracture.
Frequently Asked Questions
Can there be a fracture without swelling?
Yes — especially in hairline fractures. However, if the patient can walk 4 steps without severe pain, the likelihood is low.
When should I definitely get an X-ray?
If the patient cannot bear weight at all, or if there is tenderness over the bony prominences, an X-ray is recommended.
Is ice or heat better for a sprained ankle?
Ice is recommended for the first 48 hours to reduce swelling and inflammation. Heat should be avoided initially.
How long does a sprain take to heal?
Mild sprains typically heal within 1-2 weeks with rest and care. More severe sprains may take 4-6 weeks.
